Welcome Back! 



  • Your email address is your username. There is a link to reset your password if you do not know it. 

  • Once logged in, you should see the Dashboard for your family. You will need to complete the re-enrollment form for each student.  

  • Enter all the information that has a red * next to it on each tab. Update any information that has changed-- please review carefully!

  • ***** Attention: ALL PHONE NUMBERS MUST LOOK LIKE THIS (000) 000-000. If you do not use the (   ) and the – correctly, it will not let you move on. If you get an error, it’s probably for the phone number. 

  • On the Uploads Tab, please upload a copy of your students' completed Physician's Clearance Form.  Submit on the last tab when complete.

  • Return to the Dashboard and repeat this process for each student, including any new siblings applying. EACH STUDENT SHOULD HAVE AN ENROLLMENT FORM. Please add a new form for any new students. 

  • You can return to your family dashboard and make changes or upload documents at any time!

  • There are three requirements to apply for the Horizons summer program.

  1. Complete the online application (Link Below)

  2. Upload your Physicians Clearance form (Click the PDF to download)                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                                   

  3. Upload your Free and Reduced Lunch application (*If your child does not receive Free and Reduced Lunch, please contact us!)

  • Incomplete submissions will not be considered, so please be sure to complete all three steps!  We will contact you if your child has been accepted.
